Maddie's Legacy.
Madison's inquest finally took place on the 19th and 20th December 2006 nearly 21 months after she died. The delay was due to a lengthy police investigation and the Crown Prosecution Service (CPS) deciding whether they could bring criminal charges against the 5 people involved. A decision was made that to try these people for 'gross neglect manslaughter' would be too difficult to stand in court as the circumstances surrounding Maddie's death were so complex.
Following Maddie's inquest and the hospitals own investigation into her death they have implemented numerous changes to try to prevent this tragedy happening again. One of the main changes which WE forced through was the introduction of a '2 box system' on the prescription sheet. This means in the 1st box is the numerical value and in the 2nd box is the dosage. This way it doesn't matter whether the word 'u' or 'units' is written. We found out that Great Ormond Street Hospital used this system and pressured Alder Hey to use the same.
Also we were contacted by a doctor working for Liam Donaldson the Chief Medical Officer in the UK. He would like to use Maddie's tragic story in conferences and speeches to other medical professionals to highlight what happened, how it happened and how it could of been avoided.
We now have two beautiful young sisters for Maddie who as they grow older will know all about their big sister and her special place in this world.
